What is CHC?

Continuing Healthcare (CHC) is a package of care funded entirely by the NHS for individuals who have significant ongoing healthcare needs. It covers a wide range of support, including care at home, residential care, or nursing home care, depending on a person’s needs.

CHC is different from standard social care funding because it is based on healthcare needs rather than financial means. This means it’s available to those who meet the clinical criteria, regardless of savings or income.


Who is Eligible?

Eligibility for CHC is determined through a comprehensive assessment by the NHS, looking at your physical, psychological, and social needs. You might be eligible if you:

  • Have a primary health need that requires ongoing nursing, medical, or therapy input.

  • Have complex health conditions that need constant monitoring or support.

  • Are receiving end-of-life care.

People with long-term conditions, severe disabilities, or progressive illnesses may qualify, but eligibility is always assessed individually.


How to Claim CHC

  1. Request an assessment – You yourself, a family member, or a healthcare professional can request a CHC assessment through your GP, healthcare provider or local NHS Clinical Commissioning Group (CCG) or Integrated Care Board (ICB).

  2. Multidisciplinary assessment – A team of healthcare professionals will review your needs and complete the Decision Support Tool (DST).

  3. Decision – The NHS will decide whether you meet the criteria for CHC. You will be informed in writing and given a chance to discuss the outcome.

  4. Appeal if necessary – If CHC is declined, you can request a review or appeal. Support from a healthcare advocate or solicitor can be helpful at this stage.

It’s important to gather detailed evidence of your healthcare needs, including medical records, care plans, and any reports from therapists or specialists.
